Please include as much detail as possible.

 

For example, if there was an issue checking out an item to a student, include the item id number and the student id number involved.

 

Please keep personally identifying information for students to a minimum as much as possible. In most cases a student id number will suffice, rather than a name. In cases where we need that information, we will request it from you.
